How long does it take?
Upon receiving the complete application package, the MSP will review it within 30 days. Once approved, the registry card will be sent to the patient. If denied, a letter will be sent with details explaining why.
How do I qualify?
- Must be diagnosed with any of the qualifing conditions
- Must fill out application form
- Must submit medical records for proof
- Must submit driver’s license or state ID card. Temporary IDs are not accepted
