Basic Legal Requirements
- Must be a resident of New Mexico
- Must be 18 years of age or above.
Proof of Qualifying Conditions
- Must be suffering or undergoing treatment of a qualifying condition.
- Release of Information / recommendation from a physician.
- Physician’s written certification that the patient is suffering from a qualifying condition.
Complete the Paperwork
- Must have filled out application form
- Must submit driver’s license, release of information, and medical records.
